Importance of pharmaceutical care in compounding drug as a professional pharmaceutical service

Introduction: Community pharmacy must guarantee a suitable professional compounding drug dispensing service in which two phases are differentiated. Phase one involves assessing the prescription, collecting the necessary data to guarantee safety and effectiveness of the preparation and selecting the appropriate excipient. Phase two involves handing the preparation to the patient with oral and written information in order to guarantee correct identification, conservation and use.
Objectives: Analyze necessary data collected when the prescription for a compounding drug is received and to what extent the patient is informed about correct conservation and use. Assess the results obtained, in order to determine corrective actions and improvement opportunities in the dispensing process of individualized medication.
Method: Observational cross-sectional observational study was carried out by means of a 10-item questionnaire sent to the community pharmacists of Bizkaia to analyze actions in the two phases of dispensing and manufacturing requests to third parties.
Results: 91.9% ask for whom the medication is for, 6.1 % never ask about the illness, and 12.4% never ask about dosage. 91.4% always hand the patient information leaflet and 5.2 % never perform oral reinforcement.
Conclusions: Most community pharmacists in Bizkaia perform a good professional practice in both phases of compounding drug dispensing process, but there is great room for improvement, mainly regarding information about the disease to be treated and its localization, as well as in oral reinforcement when handing over the compounding drug. The training interventions carried out to date have improved professional practice, but further actions are needed to optimize the dispensing of individualized drug.
