A 36-year-old woman with insomnia, pain secondary to grade 3 lumbar spondylolisthesis and a history of anxiety. At the patient's request, a gradual benzodiazepine withdrawal plan was designed, based on conversion to diazepam, stabilization and a progressive dose reduction following clinical guidelines, within the framework of Comprehensive Medication Management. This case highlights the importance of a biopsychosocial approach in pharmaceutical care for patients using benzodiazepines chronically, evaluating agreed-upon alternatives and minimizing adverse effects during tapering. Collaboration between Community Pharmacy and Primary Care facilitates personalized interventions, achieving therapeutic goals and improving the patient's quality of life.
