Abstract

Introduction: This study evaluates the impact of Comprehensive Medication Management on a group of five patients attended at San Francisco Javier Pharmacy (Arrecife, Lanzarote).

Method: A mixed-methods, observational, and prospective study was conducted over a maximum period of six months. Data was collected through in-depth interviews, review of pharmaceutical records, and clinical assessments. Drug therapy problems were identified, and individualized interventions were designed for each patient. Quality of life was also assessed using EQ-5D-5L, EQ-VAS, and QALYs.

Results: Twenty-one drug therapy problems were identified in 5 patients, of which 20 (95.2%) were resolved through medication discontinuation, substitutions, dose adjustments, and health education. The most frequent were unmet needs for additional treatment (23.8%), unnecessary medication (19.0%), and adverse drug reactions (14.3%). Clinical evaluation showed nine resolved conditions, fourteen stable, six improved, three partially improved, and one worsened, including two new conditions identified during follow-up. The medication experience was predominantly positive, with good understanding of treatment and health-oriented motivations, although some concerns regarding dependency and drug efficacy were noted. Quality of life improved in all patients, with Patient 4 showing a 13.3 QALY gain and reduced anxiety/depression.

Conclusion: Comprehensive Medication Management proved effective in resolving drug therapy problems, improving clinical outcomes, and optimizing the medication experience. The results highlight its potential as both a clinical and humanistic tool, emphasizing the importance of interdisciplinary collaboration in patient-centered pharmaceutical care.
