Abstract

Objective: To create a network of active patients (PA) with the same chronic condition to promote self-confidence, self-care, and patient empowerment in self-management of their disease.

Method: Observational, cross-sectional, retrospective study of the development of the educational program “Xarxa Pacients” aimed at active patients with chronic pathologies promoted by the Very Illustrious College of Pharmacists of Valencia (MICOF) between March 2022 and June 2024. After the study of previous experiences and the selection of pathologies with the collaboration of Patient Associations and pharmacists, educational materials and a website with information on the pathologies and training calendar were developed. Two training modalities: courses, given by peers, and complementary training given by healthcare professionals. The courses resulted in patients who voluntarily became patient trainers for successive editions. At the end of the course, the active patients completed a satisfaction questionnaire. 

Results: From March 2022 to June 2024, 24 courses and 19 complementary trainings were carried out and 1,393 active patients, 61 healthcare professionals and 27 patient trainers participated. 6 pathologies included: diabetes, renal disease, fibromyalgia, chronic pain, cancer and blood diseases. The mean age in courses was 52.2 (SD=11.7) years and in complementary training 53.8 (SD=14.4) years, mostly women. The mean rating of the courses was 8.8 (SD=1.8) and of the HF was 9.3 (SD=1.2).

Conclusion: The assessment of the training sessions by active patients reinforces the usefulness of the learning methodology for improving knowledge of the disease, self-care skills and self-management.
